YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.Inspector Yee investigated the complaint at the subject property and observed violation(s) of the San Francisco Housing Code which are delineated within the Notice of Violation issued. Pertinent observations are as follows: See report.
2REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING. (703 & 1301- SFHC) PROPER CONTAINMENT AND A 3-DAY NOTIFICATION (TO RESIDENTS & ADJACENT NEIGHBORS) IS REQUIRED FOR LEAD BASED PAINT PROJECTS OF MORE THAN (10) TEN SQUARE FEET. (Sec.3604, SFBC) REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING. (703 & 1301- SFHC)

Remove and properly dispose of peeling and flaking paint on the exterior surfaces. Follow proper removal and abatement, apply a new coat of weather proofing - re-paint.

Note: It is the owner's responsibility to investigate for possible presence of lead based paint and remove following procedures set forth by the San Francisco Health and Building Departments. Consult with a qualified lead abatement contractor as required.
3INSPECTOR COMMENTSIt is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector.

YOUR COOPERATION IS APPRECIATED.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 12 July 2016 10:00:00 AM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
