YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.COMMON AREAS OF 627 PAGE ST.:
2REPAIR STAIRS (1001(b)(13)HC)EXTERIOR STAIRS USED AS EMERGENCY EGRESS, CENTRALLY LOCATED: Wood deterioration was noted on joist at intermediate landing. Repair/replace stair components as required.
A BUILDING PERMIT IS REQUIRED.

NOTE: The items of correction listed herein are those which were observed, and do not include deterioration/damage which was not easily accessable &/or may be concealed & become evident when work is begun.
3BUILDING PERMIT REQUIRED (301 HC)YOU ARE REQUIRED TO OBTAIN A BUILDING PERMIT FOR STAIR REPAIR.
When all work is complete the District Building Inspector must inspect & sign the job card before the Housing Inspector can abate this violation.
Please present the signed/finalized job card & the building permit application (pink) to the Housing Inspector at reinspection.

NOTE: The BUILDING INSPECTOR is DIFFERENT than the HOUSING INSPECTOR.
PLEASE call 415.558.6096 to schedule an appointment with the Building Inspector BEFORE the reinspection date with the Housing Inspector (SEE BOTTOM OF NOTICE FOR REINSPECTION DATE)
4REPAIR SOURCE OF WATER DAMAGE (1001(a),(b)(13),(d),(f),(l) HC)REAR STORAGE RM: An active leak was observed in the ceiling. YOU MUST LOCATE SOURCE OF WATER & make repairs as required to stop the flow of water. After leak has been stopped, then repair ceiling as required.

NOTE: If work becomes extensive &/or repairs to structure are made, then you are required to obtain a PLUMBING/BUILDING PERMIT.
5RECHARGE ALL FIRE EXTINGUISHERS (905, 1001(m) HC) ALL FIRE EXTINGUISHERS: Although fire extinguishers were current at time of inspection, it will be necessary to have them recharged & tagged before the reinspection date. (SEE REINSPECTION DATE AT END OF NOTICE)
6PERMANENTLY REMOVE COMBUSTIBLE STORAGE OR PROVIDE AUTOMATIC FIRE SPRINKLERS. (904, 1001(i) HC)GARAGE: Remove all combustible items from garage as required.
NO STORAGE allowed in garage w/out benefit of sprinklers.

NOTE: If sprinklers are installed, then storage is limited to areas which is NOT designated for parked cars.
A BUILDING/PLUMBING PERMIT IS REQUIRED IN ORDER TO INSURE PROPER INSTALLATION OF SPRINKLERS (103.1.1 PC). When all work is complete the Plumbing Inspector must inspect & sign the final inspection/job card, before the Housing Inspector can abate this violation. Please present the signed job card to the Housing Inspector at the final reinspection.
7REPAIR SMOKE ENCLOSURE DOOR CLOSER (807 HC)ROOF ACCESS DOOR: Repair/replace door closer as required. Door must close & latch without any outside assistance.
NOTE: IF door is replaced, the door must be 1-hr rated fire door, OR it must be covered w/ tin or other approved metal on ROOF side and ALL edges (A BUILDING PERMIT is required to install a new door)
8REPAIR SMOKE ENCLOSURE DOOR (807 HC)ROOF ACCESS DOOR: Provide lock on door as required.
Doors must have door handle and may be fastened on the inside w/ a simple latch, e.g. an eyehook latch or movable slide-bolt latch (NOT to be confused with a deadbolt lock. Deadbolt locks are NOT allowed)
If a keyed lock is installed it MUST be a deadlatch lock and must be keyed on the ROOF SIDE. With the installation of a deadlatch, the door must open automatically or with a thumblatch from inside of door. (Door must close & latch automatically)
9REPAIR GARBAGE CHUTE (1001(b), 1304 HC)GARBAGE CHUTE: Repair/replace missing sections of garbage chute below hopper on 2nd floor as required. (DUCT TAPE OR METAL TAPE IS NOT ACCEPTABLE)
Repair/replace hopper door at top floor of garbage chute. Doors should be gravity operated hinged or self-closing covers having a fire-protection rating.
10REPAIR FIRE PROOFING WITH APPROVED MATERIALS (1001(n) HC)EMERGENCY EGRESS CORRIDOR: Provide missing fireproofing at ceiling &/or walls as required. If sheetrock is utilized, then you must install 5/8" Type X sheet rock & tape & mud as required to maintain smoke & fire rating.
11REPAIR FIRE PROOFING WITH APPROVED MATERIALS (1001(n) HC)REAR STORAGE RM: A continuous leak was observed from the clg in this area. ***YOU MUST LOCATE & REPAIR SOURCE OF WATER & its subsequent DAMAGE BEFORE MAKING fireproofing REPAIRS*** If work becomes extensive a plumbing &/or building permit is required.

NOTE: You are required to verify the extent of water damage to wood structure in ceiling and make the necessary repairs to structure before proceeding w/ fireproofing. A BUILDING PERMIT IS REQUIRED.

After repairs are made you are required to provide missing fireproofing at ceiling as required. If sheetrock is utilized, then install 5/8" Type X gyp.bd. & tape & mud as required to maintain smoke & fire rating.
12PERMANENTLY REMOVE COMBUSTIBLE STORAGE OR PROVIDE AUTOMATIC FIRE SPRINKLERS. (904, 1001(i) HC)FURNACE/BOILER ROOM: Remove all combustible items in this room.
13INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner?s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.
14INSPECTOR COMMENTSNOTE: In order to avoid telephone tag, please do NOT leave phone messages to request alternate reinspection dates. Please call & speak with me directly to reschedule a reinspection. Otherwise feel free to leave messages regarding all other matters.

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
