YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:

1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.UNIT #308 & COMMON AREAS OF 308 31ST AVE.:
2REPLACE BROKEN GLASS WINDOW PANE (1001(h),708 HC)FRONT BEDROOM @ APT.#308: Replace cracked window glazing as required.
3REPAIR LATCHES/HARDWARE & FRAMES (1001(b),(h),(j),& 708 HC)WINDOWS IN REAR BEDROOM & BATHROOM: When working properly ALL windows must:
1. Open & close fully & easily.
2. Stay open w/out outside assistance.
3. Latch or lock.
4. Replace any cracked or broken glazing
5. Be repaired &/or adjusted so that window sash is in good condition & fits securely in window casing to prevent outside air &/or water from entering interior.

NOTE: IF WINDOWS ARE REPLACED then a building permit is required. When all work is complete the District Building Inspector must inspect & sign the final inspection/job card before the Housing Inspector can abate this violation. Please present the signed/finalized job card & application to the Housing Inspector at reinspection.

PLEASE NOTE: The BUILDING INSPECTOR is DIFFERENT than the HOUSING INSPECTOR (The Housing Inspector is the person who's name appears at the bottom of this Notice)
PLEASE CALL 415.558.6096 to schedule an appointment with the Building Inspector BEFORE the reinspection date with the Housing Inspector (SEE BOTTOM OF NOTICE FOR REINSPECTION DATE)
4REPAIR DOORS(1001(h), 706(b) HC)SLIDING DOORS BETWEEN LIVING ROOM & REAR BEDROOM @ APT.#308: Repair doors as required so doors are tight-fitting and open & close easily.
NOTE: If doors are replaced, then a BUILDING PERMIT IS REQUIRED.
5REPAIR FENCE (1001(b)(13) HC)FENCE @ SIDEWALK: Wood deterioration was noted through-out. Repair or remove as required.
6PROVIDE BUILDING ADDRESS (706(b)(11)HC)Provide building numbers as per the official street number assignment. Then place the numbers on the building in such a position that the number is easily visible to approaching emergency vehicles. The numerals shall not be less than 4 inches in height and shall be a contrasting color to the background. All numbers must be made of substantial and permanent material and must be so affixed as not to easily effaced or removed.
7REMOVE OBSTRUCTIONS FROM MEANS OF EGRESS (801,1001(l) HC; 1203 SFFC)SIDE GATE USED AS EMERGENCY EGRESS: Remove all garbage stacked in front of EXIT OPENING as required. Keep a path of travel to the street clear & unobstructed at ALL times.
8PERMANENTLY REMOVE COMBUSTIBLE STORAGE (904, 1001(i)(k), 810 HC)REAR STAIRS USED AS EMERGENCY EGRESS: Relocate garbage & remove ALL combustibles
NO STORAGE ALLOWED UNDER STAIRS/DECKS AT ANY TIME.
9RECHARGE/DATE TAG FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT (905,1001(m)HC)ALL FIRE EXTINGUISHERS: You are required to recharge fire extinguishers ANNUALLY and provide TAGS certified by the Fire Marshal INDICATING DATE that they were recharged.
10REPAIR SMOKE ENCLOSURE DOOR CLOSER (807 HC)ROOF ACCESS DOOR: Provide lock on door as required. Doors must have door handle and may be fastened on the inside w/ a simple latch, e.g. an eyehook latch or movable slide-bolt latch (NOT to be confused with a deadbolt lock. Deadbolt locks are NOT allowed)
If a keyed lock is installed it MUST be a deadlatch lock and must be keyed on the ROOF SIDE. With the installation of a deadlatch, the door must open automatically or with a thumblatch from inside of door. (Door must close & latch automatically)
11REPAIR SMOKE ENCLOSURE DOOR CLOSER (807 HC)ROOF ACCESS DOOR: Repair/replace door closer as required. Door must close & LATCH without any outside assistance.
NOTE: IF door is replaced, then it must be covered w/ tin or other approved metal on ROOF side and ALL edges, OR NEW door must be 1-hr rated fire door (A BUILDING PERMIT is required to install new door)
12PROVIDE ADEQUATE LIGHTING (504(g), 1001(b)(10) HC)REAR STAIRS USED AS EMERGENCY EGRESSS: Provide fixtures as required to illuminate egress as required.

YOU ARE REQUIRED TO OBTAIN AN ELECTRICAL PERMIT TO INSURE PROPER INSTALLATION OF LIGHTING: A copy of this notice is required to be submitted with the electrical permit application. When work is complete the electrical inspector must inspect and sign the final inspection/job card, before the Housing Inspector can abate this violation. Please present the signed job card to the Housing Inspector at the time of reinspection.
13PROVIDE ADEQUATE LIGHTING (504g HC)EXTERIOR SIDEWALK USED AS EMERGENCY EGRESS: YOU ARE REQUIRED TO PROVIDE LAMPS ON A ROUTINE BASIS & verify that all fixtures & lamps are in good working order at reinspection.
14REPAIR SINK FAUCET (505(d)(g), 1001(b-13) HC)(505(c)(g), 1001(b-13) HC)BATHROOM SINK @ APT.#308: Repair/replace faucets as required.
15REPAIR HAZARDOUS WIRING & PROVIDE LIGHTING (1001(b)(10), 1001(e)(1) HC)REAR PORCH @ APT.#308: Repair/replace overhead loose & detaching porcelain light fixtures as required to prevent electrical shock &/or a fire hazard
16REPAIR & REPAINT DAMAGED CEILING & WALLS (1001(b), 1301 HC)FRONT BEDROOM CLOSET & KITCHEN CABINETS @ APT.#308: Repair &/or repaint ALL chipping & peeling paint at all exposed surfaces in a lead safe manner (3605 SFBC, 1301 SFHC). If work becomes extensive a building permit is required.
17Disturbing lead based paint can be EXTREMELY DANGEROUS to dwelling occupants and visitors, particularly to young children, pregnant women, pets, and to people performing work on the premises. For interior or exterior paint removal : Always wet the surface, contain and properly dispose of leaded paint. If you are unsure whether the paint is leaded, you should test it prior to performing any work. If the paint is found to contain lead, you should consult with an expert about appropriate procedures. Proper containment and 3-day notification is required for exterior jobs of more than 10 sq.ft. (Sec.3604, SFBC) Informational packets are available at (415) 558-6088. You can contact the San Francisco Childhood Lead Poisoning Prevention Program at: (415) 554-8930 for free advice. IF YOU CAUSE LEAD DUST TO BE CREATED, YOU COULD BE LIABLE FOR ANY ILLNESS CAUSED BY THE DUST. Ordinance #446-97. 
18REPAIR & REPAINT DAMAGED WALLS (1001(b), 1301 HC)BATHROOM @ APT.#308: Repair wall around electrical wall outlet & repaint as required.
19PROVIDE SELF-CLOSING DEVICES AND PRIMARY LOCKING DEVICES AT EXTERIOR DOORS (706 HC)FRONT GATE: Provide/repair door closer. Provide spring loaded deadlatch w/ minimum 1/2" throw as required.
Door must close & lock automatically without any outside assistance and unlock from the inside either automatically or with a thumblatch.
20PROVIDE SELF-CLOSING DEVICES AT EXTERIOR DOORS (706 HC)EMERGENCY EGRESS GATE: Provide/repair door closer. Provide spring loaded deadlatch w/ minimum 1/2" throw as required.
Door must close and lock without any outside assistance and unlock from the inside either automatically or with a thumblatch.
21REPAIR/REPLACE DOOR HARDWARE (1001(b.13)(d)(,j) HC)FRONT DOOR to APT.#308: Repair/replace loose door knob as required to open, close, & LATCH front door.
22PROVIDE ACCESS to COMPLETE ROUTINE INSPECTION (302, 303 HC)In order to abate this notice of violation a complete inspection of all common areas must be conducted. Inspection to include all other public areas including, corridors, rear stairs/decks, garages, & storage areas which were inaccessable at intitial inspection. PLEASE HAVE ALL AREAS UNLOCKED FOR REINSPECTION.
23INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner?s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.
24INSPECTOR COMMENTSNOTE: In order to avoid telephone tag, please do NOT leave phone messages to request alternate reinspection dates. Please call & speak with me directly to reschedule a reinspection. Otherwise feel free to leave messages regarding all other matters.

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
