YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:

1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.APT. @ 2ND STORY OF 630 40TH AVE.:
2REPAIR STAIRS (1001(b)(13)HC)FRONT EXTERIOR STAIRS: Brick treads & risers separating from main building structure & freestanding stucco "handrail".
Repair/replace stair components as required. A BUILDING PERMIT IS REQUIRED.
Please note: the items of correction listed herein are those which were observed, and do not include areas that were not easily accessable and/or areas where deterioration/damage may be concealed & become evident when work is begun.
3BUILDING PERMIT REQUIRED (301 HC)YOU ARE REQUIRED TO OBTAIN A BUILDING PERMIT FOR THE REPAIR OF THE STAIRS. When all work is complete the District Building Inspector must inspect & sign the job card before the Housing Inspector can abate this violation. Please present the signed/finalized job card & application to the Housing Inspector at reinspection.
NOTE: The BUILDING INSPECTOR is DIFFERENT than the HOUSING INSPECTOR.
PLEASE call 415.558.6096 to schedule an appointment with the Building Inspector BEFORE the reinspection date with the Housing Inspector (SEE BOTTOM OF NOTICE FOR REINSPECTION DATE)
IF NEEDED, PLEASE CALL THE DISTRICT BUILDING INSPECTOR (415.558.6096) FOR CONSULTATION BEFORE COMMENCING WORK.
4REPAIR SMOKE DETECTOR (908 911 HC)CORRIDOR: Repair/replace smoke detector as required. Verify smoke detector is operational at reinspection
YOU ARE REQUIRED TO HAVE SMOKE DETECTOR WORKING @ FIRST REINSPECTION (SEE DATE NOTED BELOW) ALL OTHER WORK MUST BE COMPLETE WITHIN (30) DAYS.
***NOTE: YOU ARE RESPONSIBLE FOR PROPER MAINTENANCE OF SMOKE DETECTORS with batteries***
5REPAIR HAZARDOUS WIRING (1001(e.1) HC)KITCHEN: Provide cover plate for electric light switch.
6PROVIDE PLUMBING PERMIT (103.1.1 PC)GARAGE: Water heaters do NOT appear to have been installed w/ permit, because one water heater sits directly on garage floor and doesn't have pressure relief drain pipe installed correctly.

YOU ARE REQUIRED TO OBTAIN A PLUMBING PERMIT as required to insure proper installation of water heaters, specifically the following items:
1.Repair/replace improperly installed vents/flues.
2.Provide pressure relief drain pipes.
3.Provide additional earthquake strapping as required.
4.Relocate water heater 18" from floor. Provide stand as required.

A copy of this notice is required to be submitted with the plumbing permit application. When work is complete the Plumbing Inspector must inspect and sign the final inspection/job card, BEFORE the Housing Inspector can abate this violation.
Please present the signed job card to the Housing Inspector at the time of reinspection.
7REMOVE CORD WIRING & REPAIR HAZARDOUS WIRING (1001(e.1) HC)GARAGE: Provide cover plates for exposed wires in ALL electrical j-boxes at ceiling.
Remove ALL unapproved electrical cord wiring through-out garage, especially for laundry appliances.
8PROVIDE LIGHTING (1001(b)(10), 1001(e)(1) HC)GARAGE: Remove all cord wiring & provide light in garage as required.
Note: if work becomes extensive, then a permit will be required.
9CLEAN OR SANITIZE (1306, 1310, 1001(b.12),(d),(i),(k) HC)COMMON CORRIDOR & GARAGE: Clean debris & rubbish as required to prevent possible health hazards & potential fire hazard. Keep these areas clean and free from all debris, garbage, & all other offensive matter at all times.
Maintain on a regular basis as required for sanitary & safety reasons.
10REPAIR & REPAINT DAMAGED WALLS (1001(b)(f), 1301 HC)CLOSET IN CORRIDOR: Patch & repaint damaged wall as required.
11PROVIDE HEAT AS REQUIRED (1001(b)(6)HC) REPAIR/REPLACE HEAT SOURCE AS REQUIRED. You are required to repair inoperable wall heater as required. Owner is required to demonstrate at reinspection that all habitable rooms are capable of reaching & maintaining 68 degrees F.

NOTE: YOU ARE REQUIRED TO OBTAIN THE APPROPRIATE PLUMBING/ELECTRICAL PERMITS TO REPLACE AND/OR INSTALL NEW HEATING UNITS TO COMPLETE WORK. When all work is complete the electrical/plumbing inspectors will need to inspect and sign the final inspetion/job card, before the Housing Inspector can abate this violation.
Please present the finalized job card AND permit application to the Housing Inspector at reinspection (SEE DATE NOTED BELOW).

YOU ARE REQUIRED TO PROVIDE HEAT WITHIN 4 DAYS (SEE DATE NOTED BELOW) ALL OTHER WORK MUST BE COMPLETE WITHIN (30) DAYS.
12INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner?s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.
13INSPECTOR COMMENTSNOTE: In order to avoid telephone tag, please do NOT leave phone messages to request alternate reinspection dates. Please call & speak with me directly to reschedule a reinspection. Otherwise feel free to leave messages regarding all other matters.

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
