YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.This Notice of Violation describes violations found at the common areas of the subject property located at 1781 15th St.
2REPAIR WINDOW SASH FRAME (1001(h),708 HC) REPAIR WINDOW LATCHES OR HARDWARE (1001(b)HC) It was noted at the time of inspection that some windows could not be opened.

Repair the windows in an approved manner so that they open easily, remain open, close easily and secure shut.

IF WINDOW ASSEMBLIES ARE REPLACED WITH NEW WINDOWS A BUILDING PERMIT IS REQUIRED. INDIVIDUAL WINDOW SASH MAY BE REPLACED IN KIND WITHOUT A BUILDING PERMIT.
3REMOVE/COVER EXPOSED WIRING (1001-e HC)Exposed wires were noted at the ceiling near the bathroom door. Replace the fixture that was at this location or correctly terminate the wiring.
4MAKE NECESSARY REPAIRS TO TUB/SHOWER (1001 HC)Make necessary repairs/replacements to the shower as needed. It was noted at the time of inspection that the shower leaks at the head and taps when running.
5ELIMINATE RODENT INFESTATION (1001b, 1306 HC)Initiate or increase frequency of professional pest control services to obtain more immediate results in the elimination of infestation. Show proof of services at time of reinspection.
6PAINT CEILINGS (1001b,1301 HC)Paint the area that was repaired on the ceiling and as needed.
7ELIMINATE MOLD/MILDEW (1301,1001b(13),1306 HC)Eliminate the mold/mildew at the ceiling above the shower and as needed in the unit.
8REPAIR FLOOR COVERING (1001b,j,o HC)Repair/replace flooring as needed. It was noted at the time of inspection that the carpet near the kitchen was matted and not cleanable and that several tiles on the kitchen floor were cracked and broken. Provide approved flooring in these areas.
9PROVIDE WEATHERSTRIP FOR DOOR (1212 HC)Repair/replace the weatherstripping for the front door to the unit. At the time of inspection it was noted that the strip did not reach the carpet.
10PROVIDE DEADLATCH LOCKS WITH MIN 1/2" THROW AT EXIT DOORS (706 HC)Provide a functional doorknob with deadlatch lock. At the time of inspection the lock would not latch with the door frame.
11INSPECTOR COMMENTSDue to the violations noted, this property constitutes a Nuisance per the San Francisco Housing Code Section 1001(d)

It is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.
If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 12 April 2018 11:00:00 AM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
