YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.1135-1139 Guerrero St Apartments and Common Area.
2PROVIDE DEADLATCH LOCKS WITH MIN 1/2" THROW AT EXIT DOORS (706 HC)Trade Door. Primary locking device shall have a minimum half inch dead-latch (spring-latch), self latching or locking and openable from the interior with no key or special effort or knowlege. At the time of inspection the door was getting loked from the inside when the door was opened from the exterior with a key. The lock mechanism appears to be malfunctioning. Repair or replace the lock and provide unimpeded egress.
3FIRE EXTINGUISHER (905) HC Provide Fire Extinguisher Type 2A 10BC or equivalent with current date tag at each floor. Note: Only fire Extinguishers certified or serviced by an industry professional are approved.
4AFFIDAVIT- SELF CERTIFICATION (420.4) (907.2211) (3401.6.1)SFBC SMOKE DETECTOR (911)HC (310.9.1.2)CBC CARBON MONOXIDE ALARMS IN EXISTING DWELLINGS (SEC 420.4.2)CBC (1001(d)HC Provide smoke detectors at all sleeping rooms and connecting hallways.

Provide Carbon Monoxide Alarms (CO ALARMS) in each apartment as required.

AFFIDAVIT- SELF CERTIFICATION FOR SMOKE DETECTORS & CARBON MONOXIDE ALARMS is provided. After all devices are installed and/or tested provide the completed form to the housing inspector on the re-inspection or by email.
5REPAIR OR REPLACE FLOOR COVERING (1001(b)(13)(d)(j) HC Unit 1135A. The bathroom floor covering is damaged. Repair or replace and restore the floor covering to maintain a cleanable sanitary surface.
6SMOKE DETECTOR (911)HC, (310.9.1.2)CBC Unit 1135A. The smoke detector in the hall and bedroom is not mounted as required. Relocate the S.D. and install as required.

Provide smoke detectors in every sleeping room and connecting hallway. Smoke detectors must be installed on the ceiling or wall within 12 inches of the ceiling.
7WATER HEATERS (1301 UPC) HAZARDOUS MECHANICAL EQUIPMENT (1001(d)(g)HC) Unit 1137A. The water heater is improperly installed. Perform the required repairs with a plumbing permit.
8MOLD/MILDEW 1002(d)HC REPAIR DAMAGED CEILINGS/WALLS (1001(b)(d)(h)HC PAINT CEILINGS/WALLS (1001(b)(1301)HC REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING. (1301 HC) Unit 1137A.
a) Eliminate mold at the bathroom ceiling and walls.
b) Repair the damaged ceiling and wall in the bathroom
c) Repaint all repaired surfaces.
9GENERAL DILAPIDATION OR IMPROPER MAINTENANCE 1001(b)(d)(13) HC Unit 1139. Repair the malfunctioning unit entry door lock.

13DAMAGED PAINT (1301)SFHC WORK PRACTICE FOR LEAD-BASED PAINT (327)CEBC REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING.

Disturbing lead based paint can be EXTREMELY DANGEROUS to dwelling occupants and visitors, particularly to young children, pregnant women, pets, and to people performing work on the premises. If you are unsure whether the paint is leaded, you should test it prior to performing any work. If the paint is found to contain lead, you should consult with an expert about appropriate procedures. Proper containment and 3-day notification is required for exterior jobs of more than 10 sq.ft. (Sec 327 CEBC). Informational packets are available at (415) 558-6088.

14INSPECTION 303 SFHCProvide access to the locked room on the ground floor between the exit stairways.
15INSPECTION 303 SFHCRepairs cited in this Notice may require Building, Plumbing and/or Electrical permits. It is the responsibility of the owner to obtain (or have others obtain) any required permits before beginning work that requires permit(s). This case can not be abated until the housing inspector makes a final inspection to verify that all violations have been corrected and all required permits have been obtained and finalized. On the reinspection day, present to the housing inspector the Job Card, Permit Appication and Permits indicating that all the required work under permit is complete. Prior to the reinspection by the housing inspector, call building, electrical and/or plumbing inspector(s) for the required inspection(s).
16INSPECTOR COMMENTSIt is the property owner's responsibility to be present or to direct his/her representative to attend the reinspections scheduled on this Notice of Violation, for the purpose of providing entry to the inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this notice.

If the property owner cannot attend the scheduled reinspection (as specified on this notice) it is his/her responsibilty to secure a different inspection date and time with the inspector, and provide all tenants with notification as required by CALIFORNIA CIVIL CODE SECTION 1954. SAN FRANCISCO HOUSING CODE SECTION 303.(b), If any dwelling, apartment units or guest rooms are to be accessed during the reinspection.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 14 January 2019 03:00:00 PM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
