YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:

2REPAIR WINDOWS (1001(h),708 HC)In the kitchen, and in the dining room, the windows do not operate properly (they leak and/or do not close tight). Have a qualified professional make repairs so that windows operate and perform as originally installed and designed. Installation of new units will require completed Building Permits.

3REPAIR WINDOWS(1001(h),708 HC)In the living room, the south window and the windows to the north of the south window do not operate/close in the required manner (casement windows). Have a qualified professional, using approved methods and materials, make repairs/perform maintenance so that all 5 windows open and close as originally designed. All hardware must operate as designed, and in the closed position, a maximum gap of 3/16 between the sash and the jambstop is required. Interior and exterior of sashes must be painted or finished so that wooden frames can not absorb rainwater or condensation and deteriorate. (see item below)

4PAINT WINDOWS AND FRAMES (1001h)At the living room windows, the window stools and the sashes have missing/peeling paint or varnish. Have a qualified professional, using approved methods and materials, remove all worn and peeling paint, perform necessary repairs to deteriorated wood, and repaint/revarnish so that all window components are protected from rainwater, condensation and sunlight.
5PROVIDE FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT.(905, 1001(n)HC)At each level of the building in the common area, visible and readily accessible from the path of egress, provide approved fire extinguishers with current inspection date tags. One extinguisher at each back door landing, and one extinguisher in the ground-level passageway will fulfill this requirement

6REPAIR TOILET (1001f HC)The toilet does not operate properly. The flapper valve becomes stuck in the open position. Have a qualified professional repair the toilet so that it does not waste water.
7PROVIDE PLUMBING PERMIT (103.1.1 PC)In the bathroom, the original 66" bath tub was removed, the end wall was thickened, and a 60" bath tub was installed with new waste and supply plumbing. Have a licensed professional obtain Plumbing Permits, make any necessary changes, and then have the installation approved by the plumbing inspector.

8REPLACE ELECTRICAL OUTLET (1001e HC,406.3(D)(2)NEC)In the bathroom, the vanity has been changed out, and it is now attached to the adjacent wall which holds an electrical outlet. Have a qualified professional, using approved methods and materials, replace the standard outlet with a GFCI protected outlet.
9AFFIDAVIT REQUIRED FOR CARBON MONOXIDE AND SMOKE ALARMS (SFBC 420.4,907.2.22 & 3401.6.1)Complete self certification for the installation of carbon monoxide and smoke alarms. Form must be complete without substitution. Submit to inspector at time of reinspection, or email to inspector.
10INSPECTOR COMMENTSIt is the property owner's responsibility to be present or direct his/her representative to attend the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.
If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection time/date with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 15 April 2019 03:00:00 PM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
