YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:

1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.139 Albion Street, Unit #A.
2REPLACE BROKEN GLASS WINDOW PANE (1001(h),708 HC)FRONT LIVING ROOM & KITCHEN: Replace broken window glass pane at left-side window sash frame and provide window glazing seal; replace broken window glass pane at left-side window sash frame above sink, at kitchen, and provide window glazing seal.
3REPAIR WINDOW SASH FRAME (1001(h),708 HC)LIVING ROOM, MIDDLE BEDROOM, REAR BEDROOM, KITCHEN, BATHROOM & REAR PORCH AREA: Repair all deteriorated and defective window sash frames and locking latch devices. All operable windows in Unit #A must open & close freely, must be secure and latch properly and be weathertight. Have all window assemblies at Unit #A effectively adjusted as required.

4REPLACE WINDOW SASH CORD (1001(h),708 HC)LIVING ROOM & BATHROOM: Replace defective window sash cord @ right-side window of living room; and replace defective window sash cord at bathroom window.
5PROVIDE PRESSURE RELEASE DISCHARGE COPPER PIPE THAT TERMINATES AT GROUND LEVEL . (1001 HC) At kitchen hot water heater closet of Unit #A.
6PROVIDE AN APPROVED NON-ABSORBENT IMPERVIOUS SANITARY SURFACE FOR THE KITCHEN COUNTER TOP FOR FOOD PREPARATION. SFHC (SEC. 505) AND (1001).KITCHEN: Repair or replace defective and deteriorated kitchen countertop and sink cabinet. At time of inspection, it was observed that kitchen sink cabinet and counter were deteriorated, and water leakage from sink, penetrated onto cabinet's base areas, leaving the base area moldy, mildewed and unsanitary from prolonged deterioration.
7REPAIR SINK (1001f HC)KITCHEN: Repair loose and defective faucets at kitchen sink.
8REPAIR DAMAGED WALLS & CEILINGS (1001b,h,o HC)1) MIDDLE BEDROOM: Repair damaged walls & ceiling, exposing peeling paint and plaster cracklings at middle bedroom and including closet; and 2) BATHROOM: Repair damaged walls and ceiling, exposing peeling paint and plaster cracklings at bathroom and in closet; 3) ENTRY FOYER: Repair damaged walls & ceiling, exposing plaster cracklings, peeling paint and wooden lath boards, including at ceiling and wall of closet; 4) HALLWAY/CORRIDOR: Repair damaged walls & ceiling exposing plaster cracklings throughout noted surface areas; 5) KITCHEN: Repair damaged lower wall behind kitchen sink cabinet; AND 6) REAR PORCH AREA: Repair damaged and defective walls and ceiling adjoining window sash frame and rear porch smoke enclosure door.
9PAINT CEILINGS AND WALLS & WINDOW SASH FRAMES (1001b,1301 HC)1) MIDDLE BEDROOM, 2) BATHROOM, 3) REAR BEDROOM, 4) INTERIOR CORRIDOR, 5) FRONT LIVING ROOM, 6) ENTRY FOYER, 7) KITCHEN and
8) REAR PORCH AREA.

1) Remove peeling paint, patch crackling plaster & paint damaged walls and ceiling at middle bedroom and closet; 2) remove peeling paint, patch crackling plaster & paint damaged walls at bathroom, including shower pan area and ceiling of bathroom's closet; 3) remove peeling paint, patch crackling plaster & paint damaged walls and ceiling at rear bedroom; 4) remove peeling paint, patch crackling plaster and patch exposed wood lathe boards, paint all damaged walls and all ceiling surfaces at lower and upper walls throughout interior corridor in Unit #A; and 5) remove peeling paint, patch crackling plaster, patch exposed wood lath boards of lower walls at front living room; and 6) remove peeling paint, patch crackling plaster, patch exposed wood lath boards at lower walls at entry foyer, and paint all damaged areas; 7) repair, patch and paint damaged lower wall behind kitchen sink areas AND
8) repair and paint damaged window sash frames, walls and ceiling in rear porch area.
10ELIMINATE MOLD/MILDEW ON CEILING & WALLS (1301,1001b(13),1306 HC)BATHROOM: 1) Eliminate accumulated mold and mildew at ceiling, window sash frames, shower pan areas and walls of bathroom; AND KITCHEN: 2) Eliminate accumulated mold and mildew at lower wall and flooring behind kitchen sink cabinet.
11REPAIR FLOOR COVERING (1001b,j,o HC)REAR BEDROOM, FRONT LIVING ROOM, ENTRY FOYER & MIDDLE BEDROOM:
Repair or repaint defective and deteriorated flooring surfaces at the aforementioned damaged areas in Unit #A.
12REPAIR FLOOR COVERING (1001b,j,o HC)INTERIOR CORRIDOR/HALLWAY: Repair or replace deteriorated carpeting in unit, at edges and defective middle sections, along corridor OR hallway of unit -- from the entry foyer to rear bedroom entrance of interior corridor. At time of inspection, it was observed that carpet was worn, deteriorated and requires either repair or replacement and sanitizing; and KITCHEN: Repair or replace defective and deteriorated flooring at kitchen.
13REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING. (1301 HC)ENTRY FOYER, LIVING ROOM, CORRIDOR, BEDROOMS, BATHROOM & REAR PORCH AREA: At all designated areas aforementioned in UNIT #A.
14INSPECTOR COMMENTSAT TIME OF INSPECTION, THE HOUSING INSPECTOR WILL NEED FULL ACCESS TO ALL COMMON AREAS OF THE BUILDING, INCLUDING THE GARAGES & STORAGE SPACES IN BUILDING.


It is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access
to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.

16Disturbing lead based paint can be EXTREMELY DANGEROUS to dwelling occupants and visitors, particularly to young children, pregnant women, pets, and to people performing work on the premises. For interior or exterior paint removal : Always wet the surface, contain and properly dispose of leaded paint. If you are unsure whether the paint is leaded, you should test it prior to performing any work. If the paint is found to contain lead, you should consult with an expert about appropriate procedures. Proper containment and 3-day notification is required for exterior jobs of more than 10 sq.ft. (Sec.3604, SFBC) Informational packets are available at (415) 558-6088. You can contact the San Francisco Childhood Lead Poisoning Prevention Program at: (415) 554-8930 for free advice. IF YOU CAUSE LEAD DUST TO BE CREATED, YOU COULD BE LIABLE FOR ANY ILLNESS CAUSED BY THE DUST. Ordinance #446-97.At all designated areas of Unit #A.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 18 July 2007 11:00:00 AM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
