YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED. THE FLAT LOCATED AT 436 05TH AVE:
2REPAIR DOOR (1001(d)(h), 706(b) HC)FRONT DOOR @ EXTERIOR: Provide & install weather stripping at front door as required.
NOTE: DOORS to exterior &/or to unheated spaces should be tight fitting in order to prevent air &/or water infiltration.
3REPLACE WINDOW GLAZING as REQUIRED (1001(a),(b.13),(d), (h.2),(j),(k),(l), 703, & 708 HC)WINDOWS @ FRONT ROOM OVERLOOKING STREET, MIDDLE ROOM ADJACENT TO FRONT ROOM, LIVING ROOM, AND KITCHEN: Replace ALL cracked window glazing as required.
4REPAIR WINDOW GLAZING, LATCHES/HARDWARE, SASH CORDS, & FRAMES (1001(b)(d)(h)(j) & 703, 708 HC)ALL WINDOWS THROUGH-OUT FLAT: Repair all windows as required; after repairs all windows must:

1. Open & close fully & easily.
2. Stay open w/out outside assistance.
3. Latch &/or lock easily (NOTE: All windows accessable from the exterior must have locking devices)
4. Be repaired &/or adjusted so that window sash is in good condition & fits securely in window casing to prevent outside air &/or water from entering interior.
5. Be repaired so as to keep glazing secure in window frame.

IF WINDOWS ARE REPLACED, then a building permit is required (301, 1001(a)(b.13)(d) HC)
5REPAIR FLOOR COVERING (1001(a),(b.13),(d),(j) HC)KITCHEN: Observed improperly installed vinyl floor tiles where a large section of vinyl flooring missing around the water heater and the remaining floor tiles are cracked and/or missing creating an unsanitary condition.

Remove, repair &/or replace damaged flooring as required in order to prevent a trip hazard, water infiltration & an unsanitary condition.
6REPAIR KITCHEN SINK CABINET (505(c)(d.2)(g), 1001(b.13)(d)(j) HC)KITCHEN SINK CABINET: Observed a metal cabinet for kitchen sink with rust noted through-out inside and outside creating an unsanitary condition. Repair/replace kitchen sink cabinet as required for proper sanitation.

Provide missing flanges where pipes enter wall as required to prevent moisture & vermin from entering metal cabinet.

IF BUILT-IN KITCHEN SINK CABINET IS REPLACED, then a building permit is required (301, 1001(a)(b.13)(d) HC)
7REPAIR KITCHEN CABINETS (505(c)(d.2)(g), 1001(b.13)(d)(j) HC)BUILT-IN KITCHEN CABINETS: Observed existing wood cupboards and cabinets. Doors to cupboards won't stay closed. Repair/Replace &/or Provide latches for cupboards as required.
Observed (2) cracked drawers at lower cabinet. Repair/Replace ALL built-in cabinet drawers, doors &/or supports as required in order to provide smooth operation of all drawers & doors and proper sanitation.

IF BUILT-IN CUPBOARDS/CABINETS are REPLACED, then a building permit is required (301, 1001(a)(b.13)(d) HC)
8REPAIR DOOR (1001(h), 706(b), 505(f) HC)DOUBLE DOORS BETWEEN FRONT & MIDDLE ROOMS: Observed double doors hung incorrectly and out of alignment. Repair these doors as required in order that both doors close, latch and are tight fitting.

CLOSET DOOR IN MIDDLE ROOM USED AS BEDROOM: Repair/replace door as required in order that door closes, latches and is tight fitting.

IF NEW DOORS ARE INSTALLED, then a building permit is required (301, 1001(a)(b.13)(d) HC)
9REPAIR/REPLACE DOOR HARDWARE (1001(b.13)(d)(,j) HC)MIDDLE ROOM USED AS BEDROOM: Observed damaged wall from door knob hitting it. Replace existing door stop with a new door stop in order to prevent future damage to wall.
10REPAIR & REPAINT DAMAGED WALLS & CEILINGS (1001(b)(d), 1301 SFHC)FRONT ENTRANCE HALL, MIDDLE ROOM USED AS A BEDROOM, LIVING ROOM, HALL OUTSIDE OF BATHROOM, & KITCHEN:
Repair &/or repaint ALL walls & ceilings where surfaces have recently been repaired OR where plaster &/or paint has cracked, bubbled, peeled, &/or chipped.
After repairs then re-paint as required in order that ALL walls & ceilings maintain a SMOOTH and even visual appearance upon completion.

IF PLASTER WALLS ARE REPLACED WITH SHEETROCK, then a building permit is required (301, 1001(a)(b.13)(d) HC)

REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD (3605 SFBC, 1301 SFHC): For interior or exterior paint removal: follow the approved methods of the EPA RRP or other similar lead abatement training. This structure was constructed prior to 1978 and therefore presumed to contain lead (SEE ATTACHED LEAD HAZARD WARNING)
11PROVIDE, REPAIR, &/OR REPLACE SMOKE DETECTORS &/OR CARBON MONOXIDE DETECTORS (908 & 911 SFHC; 420.4, 907.2.11 & 3401.6.1 SFBC)Provide Smoke Detectors and Carbon Monoxide Detectors and/or Repair/Replace malfunctioning Smoke/Carbon Monoxide Detectors as required in EACH Dwelling Unit.

YOU ARE REQUIRED to provide & install Battery Operated Carbon Monoxide Detectors in each DWELLING UNIT.

The CARBON MONOXIDE DETECTOR is to be centrally located in each common corridor.

SMOKE DETECTORS are to be located in each & every sleeping room (a sleeping room is any habitable room including dining rooms or living rooms, etc. that are being used for sleeping purposes.)

Battery Operated Smoke & Carbon Monoxide Detectors should be mounted on ceilings. If smoke detectors are mounted on walls, then they must be mounted as per manufacturer's specifications, but mounted no less than 4" or no more than 12" from ceiling (NFPA 72)

NOTE: YOU ARE RESPONSIBLE FOR THE ROUTINE MAINTENANCE OF ALL SMOKE DETECTORS & CARBON MONOXIDE DETECTORS with batteries. Smoke detectors & Carbon Monoxide detectors must be tested on a regular basis and all batteries/backup batteries to be replaced annually in order to maintain their working condition. Advise occupants not to disconnect the batteries.
12REPAIR SINK CABINET (505(c)(d.2)(g), 1001(b.13)(d)(j) HC)BATHROOM SINK & CABINET: Observed unworkmanlike repair that was made in order to attach bathroom sink cabinet to wall. Remove unworkmanlike repair and re-attach bathroom sink & its cabinet to wall as required in order to prevent water infiltration and an unsanitary condition.
13PROVIDE PLUMBING PERMIT FOR WATER HEATER (1001(f), 505(g) HC, & 103.1.1 PC)KITCHEN WATER HEATER: Permit research failed to reveal a permit for the installation of gas water heater. Provide plumbing permit as required in order to insure proper installation of water heater.

Provide additional earthquake strapping as required; one seismic brace to be located in upper 1/3 of tank & other brace to be located in lower 1/3 of tank (4in. above controls) Secure bracing with 1/4in. X 3in. MIN. lag screws into studs.
14REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD (3605 SFBC, 1301 SFHC)Disturbing lead based paint can be EXTREMELY DANGEROUS to dwelling occupants and visitors, particularly to young children, pregnant women, pets, and to people performing work on the premises.
For interior or exterior paint removal : Always wet the surface, contain and properly dispose of leaded paint. If you are unsure whether the paint is leaded, you should test it prior to performing any work. If the paint is found to contain lead, you should consult with an expert about appropriate procedures. Proper containment and 3-day notification is required for exterior jobs of more than 10 sq.ft. (Sec.3423, SFBC)

15BUILDING PERMIT REQUIRED (301 HC)FOR ALL BUILDING PERMITS: When all work is complete the District Building Inspector must inspect & sign the final inspection/job card before the Housing Inspector can abate this violation. Please present the signed/finalized job card & application to the Housing Inspector at reinspection.

PLEASE NOTE: The BUILDING INSPECTOR is DIFFERENT than the HOUSING INSPECTOR (The Housing Inspector is the person who's name appears at the bottom of this Notice)

16INSPECTOR COMMENTSIt's the Owner's responsibiltiy to meet the Inspector as scheduled on this Notice of Violation. (see reinspection date at end of this Notice): It's the Property Owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector to those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the Property Owner cannot attend the scheduled reinspection date, then it's the Owner's responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.
