YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1VIOLATION LOCATION 621 Larch Way
2REPAIR WINDOWS (1001(h),708 HC) Multiple Windows in unit did not function correctly.
All windows must be weatherproof and must be able to open, stay open, close and latch. If the window requires replacement, a building permit is required.
3GENERAL DILAPIDATION OR IMPROPER MAINTENANCE (1001 (b)(13)) Front door is severly dilapitated and needs to be repaired or replaced. Door is slpit around locking mechanism and doesn provide security for tenant.
4LOCATE AND REPAIR SOURCE OF WATER DAMAGE. 703 & 1001(f) SFHC Locate and repair source of water damage in an approved manner. At time of re-inspection provide written proof of work done by certified professional to determine source of leak and repair done to prevent further leakage and water damage.
5REPAIR DAMAGED WALLS (1001b,h,o HC) Repair/replace baseboards and walls damaged by water in all areas of unit.

6REPAIR ELECTRICAL OUTLET COVER PLATES (1001e HC) Provide cover plates for all outlets in unit. This includes the covers for the in wall heating units as well.
7GENERAL DILAPIDATION OR IMPROPER MAINTENANCE (1001 (b)(13)) Repair/replace caulk seal around tub.
8ELIMINATE INFESTATION (1001b, 1306 HC) Pest infestation was evident in the unit. Eliminate the pest infestation and habitat in your property. Fill all holes, gaps and cracks with solid material to eliminate the areas of intrusion. Fill the gaps around the wall furnace, range hood vent and where require.

PROVIDE REGULAR PEST CONTROL BY AN INDUSTRY PROFESSIONAL. PROVIDE A MAINTENANCE SCHEDULE AT RESINSPECTION.
9INSPECTORS COMMENTS"It is the property owner's responsibility to be present or direct his/her representative to attend the re-inspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled re-inspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector. Provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the re-inspection. IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
