CODE VIOLATION DESC : THE LEGAL USE OF THIS BUILDING IS A 3 STORY R-2 APARTMENT HOUSE, 4 DWELLING UNITS ON 2 FLOORS WITH GARAGES AND A BASEMENT, AS STATED ON BPA #08501136 ISSUED ON 04/22/1985. PRESENTLY THERE ARE FOUR DWELLING UNITS ON 2 FLOORS WITH GARAGES AND A BASEMENT. UNIT 1389 AND THE GARAGES AND BASEMENT WERE INSPECTED. THE APARTMENT KITCHEN HAS NEW APPLIANCES, CABINETS, AND OTHER FINISHES. THE BATHROOM HAS NEW PLUMBING FIXTURES, CERAMIC TILE, AND OTHER FINISHES. THERE APPEAR TO BE SOME CHANGES IN THE ORIGINAL FLOOR PLAN AT THE HALLWAY IN FRONT OF THE KITCHEN AND ALSO AT THE KITCHEN FLOOR PLAN WHEN COMPARED TO A FLOOR PLAN SUBMITTED FOR FIRE ALARM PERMIT #200304212725 APPROVED ON 04/21/2003. PERMIT RESEARCH FAILED TO PRODUCE EVIDENCE TO SHOW THAT ANY VALID PERMITS WERE ISSUED TO ALTER OR REMODEL THIS BUILDING TO ITS PRESENT USE. THE FOLLOWING LIFE HAZARDS WERE NOTED AT THE TIME OF INSPECTION: *THERE IS UNAPPROVED WIRING AND PLUMBING (1001(e) & (1001(f)HC. *THERE IS NO CERTAINTY THAT ONE HOUR FIRE RESISTANT MATERIALS WERE USED AT TIME OF CONSTRUCTION (601)HC. *PROHIBITED USES OF COOKING AND BATHROOM FACILTIES (505) (709)HC. *SUBSTANDARD BUILDING (401(10)HC.

FAILURE COMMENT DESCRIPTION :   SUBMIT A COPY OF THIS NOTICE AND TWO SETS OF PLANS WITH THE BUILDING PERMIT APPLICTION TO LEGALIZE ALTERATIONS TO THE APARTMENT UNIT OR REVERT TO THE LAST LEGAL USE. AFTER THE BUILDING PERMIT IS ISSUED, PLUMBING AND ELECTRICAL PERMITS AND INSPECTIONS MUST ALSO BE OBTAINED AND SIGNED OFF BY RESPECTIVE INSPECTORS. TO ABATE THIS NOTICE YOU MUST CONTACT A HOUSING INSPECTOR FOR A FINAL INSPECTION AND PRODUCE ALL PERMITS, PLANS, AND APPROPRIATE SIGN-OFFS BY ELECTRICAL, PLUMBING, AND BUILDING INSPECTORS AS REQUIRED.
