CODE VIOLATION DESC : Received call on DBI Emergency call out phone at 23:00 hrs on 22 May 26 from S.F.-OEM. Regarding inspecting a structure fire at 675 O'Farrell St. Fire started at 3rd floor, front unit. That unit is a total loss. Other damage: noted on all floors; broken doors & windows, water and smoke damage, and P.G.&E has disconnected all electric and gas. I posted red tags at front of building w/red cross Notice to tenants. S.F.F.D was not on scene upon my arrival. Code/Section: SFBC 102.A Monthly monitoring fee applies. CORRECTIVE ACTION:

STOP ALL WORK SFBC 104.2.4

FILE BUILDING PERMIT WITHIN 30 DAYS

(WITH PLANS) A copy of this notice must accompany the permit application

OBTAIN PERMIT WITHIN 60 DAYS AND COMPLETE ALL WORK WITHIN 90 DAYS, INCLUDING FINAL INSPECTION SIGNOFF.

CORRECT VIOLATIONS WITHIN DAYS.
