YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1PROVIDE HANDRAILS (802(c),1001(b)(13)HC)For the rear stair at the West side of the building. The top of handrails shall be 34 to 38 inches above landings and nosing of treads. Handrail projecting from a wall shall have a space of not less than 1-1/2 inches between wall and the handrail. The handgrip portion shall be 1-1/4 inches to 2 inches in cross sectional dimension or the shape shall provide an equivalent gripping surface. Ends shall be returned or shall have rounded terminations or bends.
2THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.717 - 40th Ave - Common Areas
3RECHARGE/DATE TAG FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT (905,1001(m)HC) Recharge and provide current date tags all fire extinguishers throughout the building. Extinguishers should be recharged once a year.
4REPLACE DETERIORATED FLUE PIPE AT EXTERIOR (1001f)Replace deteriorated flue pipe at the South side of the building.

**Obtain Plumbing Permit and final inspection/sign-off by Plumbing Inspector**
5REPLACE DETERIORATED VENT SCREEN AT EXTERIOR (1001a#13 HC)Replace deteriorated wire cloth screen for ventilation at the South side of the building
6PROVIDE SEISMIC BRACES/STRAPS (510.5 SFPC,1001f HC) Provide approved type of seismic braces/straps for water heaters.
7REPLACE HOLLOW CORE ENTRY DOORS WITH MIN. 3/8" PLYWOOD OR APPROVED EQUAL ON THE ALL UNITSHollow Core or Panel Entry Doors on the ALL Units must be Replaced with Solid Core Doors or Reinforced with Min. 3/8" Plywood or Approved Equal (706HC)
8STRUCTURAL MAINTENANCE (604a HC)Affidavit Required. The Department of Building Inspection urges all owners of property with exterior stairs, decks and attachments to have a licensed professional pest control contractor, general building contractor, architect or engineer inspect your deck, porch, stairs, balcony or other attachments, every 2 years to assure their continued stability and safety. Have them provide a written report stating recommended repairs, if any. (See enclosed Compliance Affidavit)
9INSPECTOR COMMENTSIt is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this notice) it is he/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.


ALL ITEMS MUST BE COMPLETED WITHIN DAYS. REINSPECTION DATE: 01 May 2008 02:00:00 PM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
