YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.Complaint inspection at 2057A regarding general maintenance items.
2COMMENT REGARDING ALL WINDOWS' ASSEMBLIES AND OPERATION IN UNIT A (1001(h),(j),& 708 HC) All operable windows must operate (open / close) freely, must latch properly and be weather tight.
Have all window assemblies at apartment A inspected and adjusted as required that includes replacement of torn cords and loose fitting within the sash frame.

Note: Evidence of water intrusion (dry water stain) was pointed out at back storage / porch area. Have window assemblies inspected and weather proofed as required. Actual faulty weather-proofing was not verified on inspection day.
3PROVIDE ADEQUATE AND PERMANENT SOURCE OF HEAT TO EACH HABITABLE ROOM CAPABLE TO MAINTAINING A MINIMUM ROOM TEMPERATURE OF 68 DEGREES F. (701c HC) Do not use plug in type of heating devises due to the unknown type of wiring, capacity or load of the electrical system.
Have the wall mounted unit in the hallway inspected for proper operation and adequate size (BTU output proportional to the flats total square footage).
Provide thermostat controls for the subject heating unit.
4REPAIR STAINED / RUSTY BATHROOM SINK (1001f HC) Tenant stated that he tried many cleaning product to remove stain but without success.
Recoat the subject sink with approved enamel product to maintain a sanitary condition or replace with new.
5REPAIR/REPLACE MALFUNCTIONING ELECTRICAL OUTLET / RECEPTACLE (504, 1001 HC) Receptacle in front sleeping room / behind the dresser.
Advise occupants to minimize the use of long extension cords.
6SECURE LOOSE RECEPTACLE (504, 1001 HC) In middle sleeping room / behind the bed.
7MODIFY HOT WATER HEATER EXHAUST VENT PIPES IN KITCHEN AREA. (1001 HC) Shorten the vertical pipe by about 4 inches to maintain a positive upward slope of 1/4" per foot.
8REPAIR DAMAGED WALLS IN KITCHEN AREA (1001b,h,o HC) Complete patching damaged kitchen walls and paint as required.
9ELIMINATE MOLD/MILDEW ON BATHROOM CEILING AND WALLS (1001b(13), 1002, 1301 HC) Use approved mildewcide to eradicate mold/mildew infestation. Prime and paint surfaces as necessary.
10REPAIR OR REPLACE FLOOR COVERING IN KITCHEN AREA (1001(b,j,o), 1306 HC) Repair or replace damaged section of flooring to maintain a sanitary condition and smooth walking surfaces.
11ADJUST DOORS STRIKE PLATES SO DOOR CAN LATCH PROPERLY (1001 HC) At door leading to middle sleeping room (off the hallway) and closet door in second sleeping room (green room)
12ADJUST SLIDING DOOR OPERATION AT FRONT SLEEPING ROOM (1001 HC) Restore the door into its working condition. Reset sliding leafs into the track or as required.
13PROVIDE 1/2" THROW DEAD LATCH OR SINGLE CYLINDER 1" THROW DEADBOLT LOCKS FOR REAR DOOR. (706 HC) Have all back doors to all units inspected and corrected to assure compliance with the SF security ordinance.


14THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.You are required to correct the following two items in public and common areas:

1. RECHARGE ALL FIRE EXTINGUISHERS TYPE '2A 10BC' OR EQUIVALENT.(905, 1001(n)HC)
All fire extinguishers are required to be recharged and serviced annually and service date tag must be current and posted by a qualified professional vendor. Check the pressure gauge periodically as occasionally the seal may leaks and the unit becomes ineffective.

2. REPAIR DAMAGED CEILING AT GARAGE AREA (1001b,h,o HC)
Patch or cover opening with similar rated materials to maintain smoke and fire rating. Fire tape all joints or cover opening with minimum 26-GA sheet metal.

15INSPECTOR COMMENTS REGARDING "FOLLOW UP" REINSPECTION: I have scheduled a reinspection of your building on Wednesday, 25-June-2008 at 01:30 pm. Reinspection of premise to include all areas referenced above.

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
