CODE VIOLATION DESC : Failure to obtain a Building Permit and comply with DAHI Complete Inspection Notice dated 3-23-87. The notice required permits to legalize the three dwelling units on four floors of occupancy or return of the building to the legal use of two units on two floors of occupancy. The owner was notified by letter dated 4-2-98 that the original permit application was lost and that she must reapply (attached). There has been no response.

FAILURE COMMENT DESCRIPTION :   Submit copies of this notice and the original notice with the Building Permit Application. After issuance of the Building Permit, obtain Plumbing and Electrical Permits and complete the work to legalize the premises. To abate this notice, present all finalized permits, approved plans and job cards to the Housing Inspector at a final reinspection.
