YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.322 Alemany Blvd. Unit #4A and common areas of subject property.
2PROVIDE VENTILATION FOR GARBAGE ROOM (707, 1001(b)(7),(8) HC)Provide ventilation in garbage room under stairwell inside garage.
3PROVIDE SECOND MEANS OF EGRESS (801,1001(l) HC)Provide/restore an approved second means of egress from unit #4 with approved plans. At time of inspection no second means of egress exists fron unit #4A to public street.
-SUBMIT TWO SETS OF FLOOR PLANS ALONG WITH A BUILDING PERMIT APPLICATION AND A COPY OF THIS NOTICE IN ORDER TO PROVIDE AN APPROVED SECOND MEANS OF EGRESS FROM UNIT #4A. SIGNED PERMITS MUST BE PRESENTED AT THE FINAL INSPECTION IN ORDER TO ABATE THIS CASE.

4PROVIDE FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT.(905, 1001(n)HC)PROVIDE FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT WITH CURRENT DATE TAG AT EACH FLOOR. NOTE: ONLY FIRE EXTINGUISHERS CERTIFIED OR SERVICED BY AN INDUSTRY PROFESSIONAL ARE APPROVED..(905, 1001(n)HC)
5REPAIR SMOKE DETECTOR (908 911 HC)Repair/replace inoperable smoke detectors in unit #4A and in common area stairwell.
6PROVIDE MINIMUM 135 DEGREE VIEWER FOR ENTRY DOORS OF FOLLOWING UNITS (MAX MOUNTING HT OF 58") (706 HC)San Francisco security ordinance requires that all building numbers be posted and visible at the exterior and all individual unit numbers be posted at unit/apartment entry doors. Each unit entry door must have a door viewer mounted at 57" - 58" from the floor.
7PROVIDE SINGLE CYLINDER DEADBOLT LOCKS WITH A MINIMUM 1" THROW FOR ENTRY DOORS OF THE FOLLOWING UNITS. (706 HC) Provide dead bolt lock to entry door of unit #4A.
8PROVIDE ACCESS to COMPLETE COMPLAINT INVESTIGATION (201(b), 303(a) HC)The Dept. of Building Inspection has received a public complaint that potentially unsafe conditions/Housing Code violations exist at the property referenced above. As a result you are required to provide access to ALL units on All floors of occupancy in order to abate this notice of violation.
PLEASE HAVE these areas UNLOCKED at REINSPECTION.
9PROVIDE ACCESS to COMPLETE ROUTINE INSPECTION (302, 303 HC)In order to abate this notice of violation, a complete inspection of common corridors, storage areas &/or garages which were inaccessable at initial inspection to be conducted.
PLEASE HAVE these areas UNLOCKED at REINSPECTION.

NOTE: No storage is allowed in garages without benefit of sprinklers.

("Newly inputed CFC#200804240595 issued on April 25, 2008 of a Residential Apartment Building which requires routine inspection" See NOV #200987125)
10INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner¿s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.
11INSPECTOR COMMENTSIf the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 19 January 2009 01:00:00 PM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
