YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.1301 40TH AVE.:
2REPAIR FIRE PROOFING WITH APPROVED MATERIALS (HC 1001(m), 809)ALL GARAGES: Provide missing fireproofing at walls &/or ceilings as required. If sheetrock is utilized, then install 5/8" Type X gyp.bd. & tape & mud as required to maintain smoke & fire rating.

NOTE: If access panels are needed for areas located above the sheetrock ceiling, then an approved fire-rated assembly is required. (Building Permit is required)
3PERMANENTLY REMOVE COMBUSTIBLE STORAGE OR PROVIDE AUTOMATIC FIRE SPRINKLERS. (904, 1001(i) HC)ALL GARAGES: Remove all combustible items from garages as required. NO STORAGE allowed in garage w/out benefit of sprinklers.

NOTE: If sprinklers are installed, then storage is limited to areas which is NOT designated for parked cars.

A BUILDING/PLUMBING PERMIT IS REQUIRED IN ORDER TO INSURE PROPER INSTALLATION OF SPRINKLERS (103.1.1 PC). When all work is complete the Building &/or Plumbing Inspector must inspect & sign the final inspection/job card, before the Housing Inspector can abate this violation. Please present the signed job card to the Housing Inspector at the final reinspection.
4CERTIFY ALARM PANEL (909(d) HC)ALARM PANEL: Although certification was current at time of inspection, it will be necessary to have the alarm panel certified again & tagged on or before the Reinspection Date. (SEE REINSPECTION DATE AT END OF NOTICE)
NOTE: YOU ARE REQUIRED TO CERTIFY ALARM PANEL ANNUALLY.

NOTE: IF ALARM PANEL IS REPLACED, then a BUILDING PERMIT is required.
5PROVIDE ACCESS to COMPLETE ROUTINE INSPECTION (302, 303 HC)In order to abate this notice of violation, a complete inspection of storage areas outside building and inside garages which were inaccessable at the reinspection to be conducted.
PLEASE HAVE these areas UNLOCKED at REINSPECTION.

NOTE: No storage is allowed in garages without benefit of sprinklers.
6INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner¿s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 23 June 2009 01:30:00 PM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
