YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.25 SHORT ST.:
2LICENSED ARCHITECT, STRUCTURAL ENGINEER, CONTRATOR, or PEST CONTROL CONTRACTOR REQUIRED TO INSPECT ALL BUILDING ATTACHMENTS (604(a), 1001(b.13)(d) HC)PRIVATE DECKS, REAR FIRE ESCAPE &/OR OTHER BUILDING ATTACHMENTS:
The Department of Building Inspection requires all owners of properties with exterior building attachments to have a licensed professional pest control contractor, general building contractor, architect or engineer inspect your building's deck, porch, stairs, guardrails, fire escapes, elevated walkways &/or any other building attachment not mentioned herein, every 5 years to assure their continued stability and safety.

After the LICENSED General Contractor, Pest Control Contractor, Architect, or Structural Engineer INSPECTS ALL BUILDING ATTACHMENTS have them fill out the attached AFFIDAVIT that ensures their stability & safety.

IF the licensed professional recommends repairs, then YOU ARE REQUIRED TO OBTAIN A BUILDING PERMIT to repair, replace, &/or remove ANY building attachments. (IF more than 50% of ANY Building Attachment is to be REPLACED, THEN YOU MUST SUBMIT TWO SETS OF PLANS with the BUILDING PERMIT Application)

IF NO work is required, then please return the ORIGINAL affidavit completed by one of the aforementioned licensed individuals to Insp.Coble BEFORE or ON the REINSPECTION DATE NOTED BELOW.

NOTE: This violation cannot be abated until receipt of Affidavit.
3BUILDING PERMIT REQUIRED (301 HC)IF REPAIRS ARE REQUIRED to ANY BUILDING ATTACHMENT then you MUST OBTAIN a BUILDING PERMIT to make repairs.

Use only decay resistant materials to repair/replace damaged structural elements to subject property.
Do NOT paint or weather-proof any new materials until AFTER inspections.

When all work is complete the District Building Inspector must inspect & sign the job card BEFORE the Housing Inspector can abate this violation.
Please present the signed/finalized job card & application to the Housing Inspector at reinspection for case closure.

NOTE: The BUILDING INSPECTOR is DIFFERENT than the HOUSING INSPECTOR.

4RECHARGE/DATE TAG FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT (905,1001(m)HC)ALL FIRE EXTINGUISHERS: Recharge ALL fire extinguishers as required.

You are required to recharge fire extinguishers ANNUALLY.

Fire extinguishers must be certified by a licensed fire extinguisher company. This company will provide tags from the Fire Marshal indicating the date that the fire extinguishers were recharged.
5PROVIDE MISSING HARDWARE &/OR REPAIR DOOR CLOSERS TO SMOKE ENCLOSURE DOORS (807, 1001(b.13)(d)(j) HC)ALL DOORS TO GARAGES: Adjust all doors, hinges, &/or closers as required. Provide missing deadlatch, and/or strikeplate as required to restore door's fire rating.

ALL Doors must close & latch without any outside assistance, & REMAIN CLOSED AT ALL TIMES.

NOTE: DOORS shall be fire-rated as required. Do NOT cut or alter fire-rated doors in any way, because to do so destroys its fire rating. If doors are replaced, then a BUILDING PERMIT IS REQUIRED.
6MOVE ALL GARBAGE & RECYCLING RECEPTACLES TO AREA OPEN TO THE SKY OR TO AN APPROVED GARBAGE ROOM/AREA WITH AUTOMATIC FIRE SPRINKLERS (904, 906(4)HC)MOVE GARBAGE RECEPTACLES to the EXISTING GARBAGE ROOM inside GARAGE. MOVE other RECYCLING RECEPTACLES to an area OPEN to the SKY OR to a NEW & APPROVED GARBAGE ROOM/AREA with automatic FIRE SPRINKLERS.

NOTE: NO GARBAGE RECEPTACLES ALLOWED IN BASEMENTS, STORAGE AREAS, UNDER STAIRS, or IN GARAGES.
7INSPECTOR COMMENTSTO OCCUPANT: It is the responsibility of the owner of this property to comply with the attached Notice of Violation. A copy has been sent to the Owner. This copy is for your information. If you have any questions, please contact the Property Owner, Manager, or Management Company.

It is also the Owner's responsibility to provide prior notification of intended entry to all affected tenants per California Civil Code 1954 (303b HC)
8INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner¿s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.
9INSPECTOR COMMENTSIf the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.
10INSPECTOR COMMENTSNOTE: Please call to reschedule a reinspection if the date at the end of this Notice is not convenient. If I do not answer the telephone, then phone the front desk @ 558.6220 and ask the attendant on duty for my whereabouts. Thank you.

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
