YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.THE GROUND FLOOR FLAT @ 113 28TH STREET:
2ELIMINATE RODENT INFESTATION (1001(b.12)(d), 1306 HC)Due to rodent problem at subject property, you are required to have a licensed exterminator inspect this building & provide a written report which recommends repairs that must be made to rat proof this building. YOU ARE THEN REQUIRED TO make the recommended repairs in order to ensure that the building is rat proofed.
PLEASE PROVIDE EXTERMINATOR'S REPORT AT REINSPECTION.
3REPAIR DAMAGED WALLS, FLOORS, CEILINGS, &/or BUILT-IN CABINETRY IN ORDER TO PREVENT RODENT INFESTATION (1001(b.12),(b.13),(d),(h), 1306 HC)GAS WALL HEATER, WATER HEATER BEHIND REFRIGERATOR, KITCHEN SINK CABINET: Provide rat proofing @ baseboards, cabinets, floors, walls, & pipe penetrations in the aforementioned areas as required in order to prevent a rodent infiltration.
4REMOVE RUBBISH OR TRASH (1304,1306,1001(b)(d) HC)OPEN CHANNEL BETWEEN HOUSE & REAR YARD: Remove excess wood debris stored in this channel. Keep this space clean and free from all accumulation of debris at all times. Maintain on a regular basis as required for sanitary & safety reasons.
5ELIMINATE MILDEW ON WALLS &/OR CEILINGS (703, 1001(b.13)(d)(f), 1301, 1306 HC)BATHROOM: Inspector observed mildew on walls &/or ceiling that appears to be caused by condensation. You are required to remove mold/mildew. It is recommended to remove mold with a MILD solution of bleach & THEN paint walls with a primer containing mildewcide.

NOTE: Although there was NO evidence of water staining on ceiling or walls, the OWNER must verify that mildew was NOT caused by water intrusion due to a structural defect. If water intrusion was the cause, then YOU MUST LOCATE SOURCE OF WATER INFILTRATION & MAKE REPAIRS AS REQUIRED BEFORE MAKING COSMETIC REPAIRS (703, 1001(b)(f) HC) If work becomes extensive a plumbing &/or building permit is required.

VERIFY that bathroom window is operational, and that it opens & closes easily in order to provide adequate ventilation & prevent future moisture/mildew in bathroom.
6REPAIR SINK / CABINET (505(c)(d.2)(g), 1001(b.13)(d)(j) HC)KITCHEN SINK / CABINET: Replace rusted panel @ bottom inside metal sink cabinet as required for proper sanitation.
Provide an approved non-absorbent material as required. Caulk & seal cabinet & pipe penetrations in order to prevent rodents entry.
7PROVIDE HEAT AS REQUIRED (1001(b.6)(d) HC)LIVING ROOM: Inspector observed non-working wall-mounted gas heater & broken thermostat on adjacent wall.
REPAIR/REPLACE heat source as required.
RELOCATE thermostat to Bedroom.

IF HEATER is REPLACED or NEW HEAT SOURCE IS INSTALLED, THEN the appropriate PERMITS ARE REQUIRED. Submit an informational sheet for the proposed heating system including heat calculations for ALL habitable rooms when applying for the Building Permit.

YOU ARE REQUIRED TO PROVIDE HEAT on or before the reinspection date. (see reinspection date at end of Notice)
Because heat is an urgent matter, NO time extension can be given.
ALL OTHER WORK MUST BE COMPLETE WITHIN (30) DAYS.

NOTE: Plug-in type heating devices are NOT ALLOWED due to the unknown capacity of existing electrical system.
8INSPECTOR COMMENTSTO OCCUPANT: It is the responsibility of the owner of this property to comply with the attached Notice of Violation. A copy has been sent to the Owner. This copy is for your information. If you have any questions, please contact the Property Owner, Manager, or Management Company.

It is also the Owner's responsibility to provide prior notification of intended entry to all affected tenants per California Civil Code 1954 (303b HC)
9INSPECTOR COMMENTSOWNER'S RESPONSIBILITY TO MEET INSPECTOR AS SCHEDULED ON THIS NOTICE OF VIOLATION (see date below): It is the property owner¿s responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.
10INSPECTOR COMMENTSIf the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b)), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.
11INSPECTOR COMMENTSNOTE: Please call & speak with me directly in order to request alternate reinspection dates. If I do not answer the telephone during these hours, then phone the front desk @ 558.6220 and ask the attendant on duty for my whereabouts.
