CODE VIOLATION DESC : NOTE: Per Notice of Violation issued by building inspector John Lee on 3/16/2000. Notice of Violation issued by Senior Building Inspector Andy Greene on 3/24/2000.Thus, owner arranged for today,s inspection, including electrical, plumbing, and building inspectors. Since our observation 4 kitchens with sinks and relocated front stair facing Acadia Street.(Suggesting conforming use? and # of legal dwelling units. )Need a building permit with elevation for City Planning Department's approval and floor plan of existing house.

FILE BUILDING PERMIT WITHIN 30 DAYS

(WITH PLANS) A copy of this notice must accompany the permit application
