2PERMANENTLY REMOVE COMBUSTIBLE STORAGE OR PROVIDE AUTOMATIC FIRE SPRINKLERS. (904, 1001(i) HC)ALL COMBUSTIBLE MATERIAL AND GARBAGE IN THE REAR YARD TO BE REMOVED.
3KEEP SMOKE ENCLOSURE DOORS CLOSED (807 HC)THE DOOR AT THE TOP OF THE STAIRS THAT LEADS ON TO THE ROOF IS TO BE REPAIRED SO THAT IT GOES TO A COMPLETELY CLOSED POSITION WHEN THE SELF-CLOSURE DEVICE OPERATES.
4PROVIDE FIRE SPRINKLERS AT BOTTOM OF GARBAGE CHUTE (906(4) HC)THE GARBAGE CHUTE IS TO BE SECURED PROPERLY TO THE BUILDING AND FIRE SPRINKLER PROVIDED AT BOTTOM OF CHUTE. THE SPRINKLER HEAD IS TO PROTECTED BY A BAFFLE SO THAT IT IS NOT DAMAGED BY FALLING GARBAGE.
5OTHER PLUMBING AND/OR ELECTRICAL VIOLATIONSTHE DRAIN PIPE AT THE REAR OF THE PROPERTY NEEDS TO BE REPAIRED AND SECURED TO THE BUILDING
6OTHER PLUMBING AND/OR ELECTRICAL VIOLATIONSTHE DRAIN IN THE PATH OF EGRESS (WHERE THE METERS ARE LOCATED) NEEDS TO BE FITTED WITH A COVER.

9PROVIDE SHUTOFF TOOL FOR GAS UTILITY SHUTOFF (712 HC)THE GAS SHUT-OFF TOOL IS TO BE PERMANENTLY STORED IN THE IMMEDIATE VICINITY OF THE GAS SHUT-OFF VALVE.
10POST ENCLOSED GAS METER INSTRUCTIONAL DIAGRAM IN A PUBLIC AREA. (712 HC)IN THE VICINITY OF THE GAS SHUT-OFF VALVE

THIS ADDENDUM WAS PREPARED AFTER THE SCHEDULED RE-INSPECTION PERFORMED ON JULY 3, AT 10:00 AM.
ALL ITEMS ON THE NOTICE OF VIOLATION DATED JUNE 2, 2000, EXCEPT FOR ITEMS #4 & 5, HAVE BEEN COMPLIED WITH. INSPECTOR IS GRANTING A ONE-TIME EXTENTION OF 15 DAYS TO COMPLETE THESE ITEMS. AT THE NEXT RE-INSPECTION, ALL ITEMS ARE TO BE COMPLETED.


ALL ITEMS MUST BE COMPLETED WITHIN 15 DAYS. REINSPECTION DATE: 18 July 2000 12:30:00 PM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
