YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.Complaint inspection at 51-A Albion Street regarding general maintenance items.

2PROVIDE ADEQUATE AND PERMANENT SOURCE OF HEAT TO EACH HABITABLE ROOM CAPABLE TO MAINTAINING A MINIMUM ROOM TEMPERATURE OF 68 DEGREES FAHRENHEIT. (701c HC) Do not use plug in type of heating devises due to the unknown type of wiring, capacity or load of the electrical system.

Restore nonworking heater in front room into its working condition. Consult with a professional heating contractor to assure that the existing heater is capable of heating the entire flat based of the unit's rating "BTU output", have them provide a heat calculation as required.
3REPAIR WINDOW SASH FRAME (1001(h),708 HC) In kitchen area and bathroom.
Note: All operable windows must operate (open / close) freely, must latch properly and be weather tight.
Have all window assemblies inspected and adjusted as required.
4GENERAL MAINTENANCE REQUIRED AT FRONT DOOR: (1001 HC) Secure loose hinges as required. Adjust door / shave edges as required for proper fitting.
Painting is required at door and frame.
5REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING. (703 & 1301 - HC) Remove and properly dispose severely peeling and flaking paint at entry hallway ceiling, back sleeping room ceiling and bathroom walls and ceiling including all woodwork.
Following proper removal and abatement, apply a new coat of paint.

Note: It is the owner's responsibility to investigate for possible presence of lead based paint and remove following procedures set forth by the San Francisco Health and Building Departments. Consult with a qualified lead abatement contractor as required.
6REPAIR ILLEGAL WIRING AT BATHROOM AND KITCHEN CEILING / LIGHTS (504, 1001 HC) Consult with a qualified electrical contractor for proper wiring and fixture installations at the subject areas.
7REPAIR FLOOR COVERING IN BATHROOM AND KITCHEN (1001(b,j,o), 1306 HC) Reset all loose linoleum tiles or replace with new to maintain a sanitary and cleanable surfaces.

Properly seal any detach seams at kitchen flooring to maintain a smooth walking surfaces, sanitary and cleanable surfaces.

Seal hole in floor under the kitchen sink.

Remove all nails left behind after removing carpeting/runner from the from hallway. Tenant acknowledged removing the runner but the nails left behind. It was recommended to the tenants to hammer the nails in for their safety.
8GENERAL MAINTENANCE REQUIRED AT BACK DOOR: (1001 HC) Egress Obstruction. Replace missing door knob at the subject door.


9PROVIDE FIRE EXTINGUISHERS TYPE 2A 10BC OR EQUIVALENT.(905, 1001(n)HC / 3.29 SFFC) A minimum of one '2-A:10-B:C (or equivalent)' extinguisher for each 3,000 square feet of floor area is required. Travel distance shall not exceed 75 feet to the nearest extinguisher.
All fire extinguishers are required to be recharged and serviced annually and date tag must be current and posted by a qualified professional vendor.
Check the pressure gauge periodically as occasionally the seal may leaks and the unit becomes ineffective.
10REPAIR FRONT WOOD STAIRS (1001(b)(13)HC) At second step from the bottom. Broken and loose step was observed at the time of inspection.
Repair or replace as required to maintain a safe condition.

11COMMENTS REGARDING POSSIBLE LEAD BASED PAINT CONTAMINATION Disturbing lead based paint can be EXTREMELY DANGEROUS to dwelling occupants and visitors, particularly to young children, pregnant women, pets, and to people performing work on the premises. For interior or exterior paint removal : Always wet the surface, contain and properly dispose of leaded paint. If you are unsure whether the paint is leaded, you should test it prior to performing any work. If the paint is found to contain lead, you should consult with an expert about appropriate procedures. Proper containment and 3-day notification is required for exterior jobs of more than 10 square feet. (Sec.3604, SFBC) Informational packets are available at (415) 558-6088.
You can contact the San Francisco Childhood Lead Poisoning Prevention Program at: (415) 554-8930 for free advice. IF YOU CAUSE LEAD DUST TO BE CREATED, YOU COULD BE LIABLE FOR ANY ILLNESS CAUSED BY THE DUST. Ordinance #446-97.

12INSPECTOR COMMENTS REGARDING "FOLLOW UP" REINSPECTION: I have scheduled a reinspection of your building on Friday, 28-December-2012 at 11:30 am. Reinspection of premise to include all areas referenced above.
