YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED. Unit # 3554 20th and common areas.
2REPLACE BROKEN GLASS WINDOW PANE (1001(h),708 HC) Unit #3554: On the date of inspection, a broken window pane adjacent to an airwell, was noted in the living room area. Replace any broken windows where they may exist.
3MEANS OF EGRESS AND MANUAL RELEASE (706, 801 HC) (1008.1.6 CBC) NUISANCE (401(2)(1001(d)HC) Common Area: On the date of inspection, an unlocked sidewalk hatch and basement level corridor is the secondary means of egress for the apartment building. Currently, this means of egress is substandard. Provide an approved means of egress for all units, and proof of a minor sidewalk encroachment permit from the Bureau of Street Use and Mapping.

Keep all exit paths clear and unobstructed. Additionally provide an approved primary locking device that is openable from the interior with no need for a key, effort, or special knowledge.
4PROVIDE FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT.(905, 1001(n)HC) On the date of inspection, no fire extinguishers were present. All fire extinguishers are required to be recharged and serviced annually and service date tag must be current and posted by a qualified professional vendor. Check the pressure gauge periodically as occasionally the seal may leaks and the unit becomes ineffective.
5REPAIR BATHROOM SINK (1001f HC) NUISANCE AND HOT WATER (401(2), 1001(d), 505d(3) HC) Unit # 3554: Make necessary repairs to the faucet located in the bathroom. Ensure it is affixed properly. Ensure that water can heat to a minimum of 105 degress F (41 Degress C.) and a maximum of 120 degress F (49 Degress C.).
6PROVIDE COVER-PLATES TO ALL RECEPTACLE AND SWITCHES (504 HC) HAZARDOUS ELECTRICAL (89-23 EC) (1001(e(1)HC) Common Area: On the date of inspection, a light switch was missing a coverplate and there was exposed electrical wiring installed in an unapproved manner at the ground level rear backyard. Permit needed.
7PROVIDE ADEQUATE LIGHTING (504g HC) Common Area: Be able to show adequate lighting in the rear area and for the secondary means of egress pathway on the date of reinspection.
8PROVIDE ELECTRICAL PERMIT (90.25 EC)NOTE: ELECTRICAL PERMIT NEEDED, MAKE SURE THAT ALL PERMITS HAVE BEEN SIGNED OFF PRIOR TO CONTACTING HOUSING INSPECTOR FOR FINAL INSPECTION (504 HC)
9CLEAN OR SANITIZE (1306 HC)Common Area: Clean or sanitize airwells. On the date of inspection, pigeon droppings and other rubbish was noted in the airwells.
10LOCATE SOURCE OF WATER DAMAGE (703, 1001f HC)Unit #3554: On the date of inspection, exposed walls were noted in the kitchen/sleeping room area, due to a possible water leak. Locate source of leak. Provide a written and signed report from a qualified plumbing company or contractor stating that plumbing is in working order.
11REPAIR DAMAGED WALLS AND CEILINGS (1001b,h,o HC) Unit #3554: On the date of inspection, exposed walls were noted in the kitchen/sleeping room area. Repair per Code.
12PROVIDE PLUMBING PERMIT (103.1.1 PC) NOTE: If a new heating system is installed, building, plumbing and /or electrical permit(s) are required or plumbing permit is required if there is the installation of new plumbing fixtures. MAKE SURE THAT ALL PERMITS HAVE BEEN SIGNED OFF PRIOR TO CONTACTING HOUSING INSPECTOR FOR FINAL INSPECTION (103.1.1 PC)
13PROVIDE 1/2" THROW DEADLATCH OR SINGLE CYLINDER 1" THROW DEADBOLT LOCKS FOR REAR OR SECONDARY DOORS OF FOLLOWING UNITS (706 HC) Unit #3554: Provide an approved locking device for the rear exit door.
14REPAIR FRONT ENTRY GATE (706, 801, 1001 HC) Unit # 3554: On the date of inspection, the front metal gate was broken within 40 inches of the primary locking device. Ensure the gate entry is burglar resistant in an approved manner.
15PROVIDE HEAT TO EACH OCCUPIED HABITABLE ROOM CAPABLE TO MAINTAINING A MIN. ROOM TEMP. OF 68 DEGREES F. (20 DEGREES C.). (701c HC) Unit 3554: On the ground floor unit, there is no permanent source of heat. Have a heating professional adjust existing heating system or install additional heating sources to meet the heating requirements. Note: All required heat sources must be permanently installed.
16INSPECTOR COMMENTSIt is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.

-Please provide access to all common areas of building on re-inspection date in order to perform a complete routine inspection of the property.


ALL ITEMS MUST BE COMPLETED WITHIN 16 DAYS. REINSPECTION DATE: 24 March 2014 10:00:00 AM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
