YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:

1	THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.	Routine inspection of Common and Public areas at 1322/1324 40th Avenue
2	REPAIR WOOD STAIRS AT THE BACK OF PROPERTY (1001(b)(13)HC)	REAR WOOD STAIRS: Wood deterioration / decay was noted at stringers and vertical support members. Some treads were either loose and/or insecure. Repair or replace these areas as required to maintain a safe and sound structure. Note: Building Permit is required to do this repairs. For total replacement of stairs or deck, then YOU MUST SUBMIT TWO SETS OF FLOOR PLANS ALONG WITH A COPY OF THIS NOTICE WITH A BUILDING PERMIT APPLICATION. NOTE: The Department of Building Inspection urges all owners of property with exterior stairs, decks and attachments to have a licensed professional pest control contractor, general building contractor, architect or engineer inspect your deck, porch, stairs, balcony or other attachments, every 2 years to assure their continued stability and safety. Have them provide a written report stating recommended repairs.
3	PROVIDE MINIMUM 135 DEGREE VIEWER FOR ENTRY DOOR OF WEST UNIT (MAX MOUNTING HT OF 58") (706 HC)
4	PROVIDE DESIGNATED MISSING UNIT/APARTMENT IDENTIFICATION NUMBERS TO ALL UNITS (706 HC)
5	INSPECTOR COMMENTS REGARDING REINSPECTION:	I have scheduled a reinspection of your building on 03/29/2002 at 09:30 am. Reinspection of premise to include all areas referenced above. Please call if you have any questions, want to reschedule or confirm the time or day of reinspection. YOUR COOPERATION IS APPRECIATED.
