YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.Unit: 2819 Folsom.

2REPAIR ALL WINDOWS INCLUDING GLAZING SEALS, SASH CORDS, SASH FRAMES AND LATCHES OR HARDWARE (1001(h),(j),& 708 HC) On the date of inspection damaged windows were noted at the middle sleeping room (child bedroom), living room (front room), and bathroom adjacent to rear sleeping room. On the date of inspection, the damage noted includes but is not limited to: deteriorated sealant, broken sash frames and cords (bathroom), and exposed/loose glass panes (bathroom). Repair/replace all windows where damage exists.

IF WINDOW ASSEMBLIES ARE REPLACED WITH NEW WINDOWS A BUILDING PERMIT IS REQUIRED. INDIVIDUAL WINDOW SASH MAY BE REPLACED IN KIND WITH OUT A BUILDING PERMIT.
3RECHARGE/DATE TAG FIRE EXTINGUISHER TYPE 2A 10BC OR EQUIVALENT (905,1001(m)HC)On the date of inspection, the fire extinguisher showed expired date tags. Reminder: all fire extinguishers are required to be recharged and serviced annually and date tag must be posted and current.
4PROVIDE SMOKE DETECTORS (908,911 HC) Repair or replace missing/inoperable smoke detectors as required. Test smoke detectors on a regular basis and replace batteries annually.
5PROVIDE A MINIMUM ONE CARBON MONIXIDE ALARM/DETECTOR PER DWELLING UNIT / APARTMENT (908, 911 HC, 420.4 SFBC) Install per manufacturer instructions and as required.
6REPAIR OR REPLACE MALFUNCTIONING STOVE IN THE KITCHEN (1001 HC) On the date of inspection, the right front burner of the stove located in the kitchen would not turn on, repair/replace to working order.
7REPAIR SINK AND TOILETS (1001f HC) On the date of inspection, the following was noted:
1) rear utility room toilet was not properly sealed at its base;
2) rear utility room toilet lacked toilet dispenser;
3) kitchen sink had an unapproved plastic p-trap (needs approved metal piping); and
4) kitchen cabinet sink had extensive rust/deterioration on the enamel of the sink, cabinet doors, and the interior of the cabinet. Ensure doors open and close with ease.
8REPAIR ELECTRICAL OUTLET AND LIGHT FIXTURES (1001e HC)On the date of inspection, the following was noted:

1)top of a duplex receptacle in the rear utility room was not in working order with a missing coverplate;
2)light fixtures throughout the Unit were rusted/deteriorated and needed approved covers such as in the rear sleeping room bathroom, living room, and rear bedroom; and
3) In the rear sleeping room and living room, conduits and junction boxes were rusted/damaged. Replace/repair.

NOTE: Electrical Permit may be needed.
9ELIMINATE MOLD/MILDEW ON CEILINGS, WINDOWS, WALLS (1301,1001b(13),1306 HC) Eliminate mold/mildew on the windows and walls where mold/mildew exists. On the date of inspection, mold/mildew was present at:
1) living room adjacent to Folsom street on the West and South interior walls;
2) child sleeping room on the North and West interior walls;
3) bathroom in rear bedroom on the walls and ceilings; and
4) rear utility area windows.
10REPAIR/REPLACE FLOOR COVERING (1001b,j,o HC) On the date of inspection, damaged linoleum was noted at the rear bathroom in the unit at the entry way and near the the toilet, in the kitchen, and near the tub at the sleeping room tub/shower. Missing thresholds were also noted near the kitchen.
11REPAIR DAMAGED WALLS/CEILINGS (1001b,h,o HC)Repair ceiling and wall damage at the following locations:
1) in rear utility bathroom ceiling;
2) in kitchen on South wall where there is damaged plaster; (recommendation: if wall opened check for structural integrity issues, consult a licensed contractor); and
3) near unit entry door.
12MAKE NECESSARY REPAIRS TO SHOWER/TUB (1001f HC) For tub/shower adjacent to rear sleeping room make necessary repairs to the damaged drain, enamel, and tub valves.
13REMOVE OR COVER DAMAGED PAINT IN AN APPROVED MANNER TO PREVENT A LEAD HAZARD. SEE ATTACHED LEAD HAZARD WARNING. (1301 HC) Remove or cover peeling paint where it exists in the Unit. In particular cover peeling paint at rear sleeping room area.
14Disturbing lead based paint can be EXTREMELY DANGEROUS to dwelling occupants and visitors, particularly to young children, pregnant women, pets, and to people performing work on the premises. For interior or exterior paint removal : Always wet the surface, contain and properly dispose of leaded paint. If you are unsure whether the paint is leaded, you should test it prior to performing any work. If the paint is found to contain lead, you should consult with an expert about appropriate procedures. Proper containment and 3-day notification is required for exterior jobs of more than 10 sq.ft. (Sec.3423, SFBC) Informational packets are available at (415) 558-6088. You can contact the San Francisco Childhood Lead Poisoning Prevention Program at: (415) 252-3800 for free advice. IF YOU CAUSE LEAD DUST TO BE CREATED, YOU COULD BE LIABLE FOR ANY ILLNESS CAUSED BY THE DUST. Ordinance #446-97.  
15REPAIR DOORS AND LOCKING DEVICES AT INTERIOR DOORS (706 HC) On the date of inspection the following was noted:
1)interior door to the middle sleeping room (child room) had damaged wood at the door near the lock, missing door knob, and a non-working locking device;
2) common area: wooden door at side corridor near yard would not open and close with ease and had damaged wood at the bottom; and
3) provide an approved locking device for rear door to unit #2819.

DOORS SHALL BE TIGHT FITTING, IN GOOD CONDITION, AND SELF CLOSING. FOR EXIT DOORS, PRIMARY LOCKING DEVICE SHALL HAVE A MINIMUM HALF INCH DEAD-LATCH (SPRING-LATCH), SELF LATCHING OR LOCKING AND OPENABLE FROM THE INTERIOR WITH NO KEY, OR SPECIAL EFFORT OR KNOWLEDGE.
16NUISANCE (401(2) (1001(d) HC THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.
17INSPECTOR COMMENTSIt is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.

If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.

-Please provide access to all common areas of building on re-inspection date in order to perform a complete routine inspection of the property.


ALL ITEMS MUST BE COMPLETED WITHIN 30 DAYS. REINSPECTION DATE: 18 May 2014 11:00:00 AM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
