YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED.Complaint Inspection at apartment #1 and
Routine Inspection of common and public areas at 1395 40th Avenue.
2COMPLETE CEILING REPAIRS AT BATHROOM ND LIVING ROOM OF APARTMENT #1 (1001b,h,o HC)Assess source of water intrusion and repair at the source. Restore ceiling into its original condition. Patch, prime and paint to match existing.
3REPLACE ALL BROKEN GLASS WINDOW PANES IN GARAGE AREA (1001(h),708 HC)In garage area along west and east walls.
4REPAIR FIRE PROOFING AT GARAGE CEILING WITH APPROVED MATERIALS (HC 1001(m), 809)Patch all ceiling holes in garage area due to water leaks. Fire tape all joints to maintain smoke and fire rating.
Assess source of water intrusion and repair as required.
Consult with a roofing contractor and check all roof drains and down spouts for proper operation.
5REPAIR, MAINTAIN FIRE ESCAPE DROP LADDER IN WORKING CONDITION (SEC 908 HC)Note:
Property manager, owner or the owner's agent needs to provide a proof of the workability of the fire escape ladders.
Non-working fire escape drop ladder is a life safety hazard and must be corrected immediately.
Provide a written report from a qualified service company or contractor stating that all parts and cables have been tested and are functional or contact the housing inspector (within 30 days) to witness lowering the ladder in person.
Reminder:
Test and verify that all windows or doors leading to emergency fire escape are operational and unobstructed.
Advise tenants not to block the bath of emergency exits on the inside.
**REMINDER**
All manufacturers recommend that the fire escape drop ladder be maintained and serviced annually, particularly the cable and all moving parts. Lubricate and drop the ladder at least once a year.
6PERMANENTLY REMOVE COMBUSTIBLE STORAGE IN GARAGE AREA OR PROVIDE AUTOMATIC FIRE SPRINKLERS. (904, 1001(i) HC)**Fire Hazard**
NOTE: garages in apartment houses shall be used only for storage of automobiles. (603 (b) HC) limit storage to designated area that is fully sprinkled.
Note: plumbing permit will be required for any automatic sprinkler system installation.
7RECHARGE ALL FIRE EXTINGUISHERS (905, 1001(m) HC) **Reminder**
All fire extinguishers are required to be recharged and serviced annually and date tag must be posted and current.
Check the pressure gauge periodically as occasionally the seal may leaks and the units becomes ineffective.
8PROVIDE SMOKE ENCLOSURE DOOR CLOSER AT DOOR SEPARATING GARAGES FROM PUBLIC HALLWAY (807,707 HC)Note: this door must remain in closed position at all times. A closer is required to maintain smoke and fire separation.
The closer must be able to close and latch the door(s) freely and without any outside assistance.
9PROVIDE BATTERY OPERATED SMOKE DETECTOR AT TOP OF STAIRS (911 HC)It is recommended that smoke detectors be tested on a regular basis and all batteries be replaced annually to maintain a working and functional device.
10REPAIR CORRODED SUPPLY DUCT AT FURNACE AREA. (1001 HC)Replace disconnected section and have all systems tested for efficiency and proper air flow. Check return air ducts for possible leaks. Consult with a qualified heating contractor.
Per occupant at apartment #1: there is unusual odor comes into the apartment through the return air opening in the hallway. Investigate and repair as required.
11REPAIR SKYLIGHT ASSEMBLY AS REQUIRED. (1001 HC)Restore skylight assembly, at top of stairs, into its original condition. Reconnect or replace missing screens. Have flashing and assembly checked to prevent water infiltration and leaks into public areas below.
12NOTE FOR OCCUPANTS INFORMATION:**DEAR OCCUPANT** Chapters 1, 2, & 3 of the San Francisco Housing Code require that the Department of Building Inspection perform periodic health & safety inspections of residential property. This Routine Inspection was conducted to assure that all requirements have been met per attached Routine Inspection Checklist. Building code section 102.3 requires that a copy of this notice of violation be provided to each tenant or occupant of the above referenced property. If there are any questions please contact the property owner, manager, or property management company.
13INSPECTOR COMMENTS REGARDING "FOLLOW UP" REINSPECTION:I have scheduled a reinspection of your building on 01/22/2003 at 12:30 pm. Reinspection of premise to include all areas referenced above. Please call if you have any questions, want to reschedule or confirm the time or day of reinspection.
