YOU ARE HEREBY ORDERED TO COMPLY WITH THE FOLLOWING REQUIREMENTS:
 ITEMDESCRIPTION
1THIS NOTICE INCLUDES VIOLATIONS FOR THE AREAS NOTED. Common Areas
2CLEAN UP STANDARDS (SFBC 3407.4.4) Visible work dust and debris was not cleaned up at the completion of work or when access to the regulated area was required. Clean up all visible paint debris within three days of the date of this Notice of Violation.
3CONTAINMENT AND BARRIER SYSTEMS (SFBC 3407.4.2) DEMOLITION (SFBC 3407.3) Lead-based paint was disturbed or removed during alteration or demolition work without containment and barrier systems or HEPA vacuum local exhaust tool. Use appropriate containment and barrier systems or HEPA vacuum local exhaust tool when creating work debris during alteration or demolition work.
4POST SIGN (SFBC 3407.5.4)The responsible party did not post a sign(s) stating that lead
work was in progress before work began that disturbs and/or
removes lead-based paint.
5INSPECTOR COMMENTSOn the date of inspection, extensive alteration work was noted at hotel (related permits: E201408081824, PP20140821418, and 201408073295). The fire safety systems including but not limited to central alarm, fire resistant passageways, and sprinklers were not in working order with live/exposed wires noted throughout property. Raw sewage was noted and an open sanitary facility on the occupied floor. Dust and construction debris noted throughout. Referrals were made to Public Health and BID.
6INSPECTOR COMMENTSIt is the property owner's responsibility to be present or direct his/her representative to attend, the reinspection as scheduled on this Notice of Violation for the purpose of providing entry to the Inspector of those areas not accessed during the initial inspection as specified, and/or to provide access to all areas cited within this Notice.
If the property owner cannot attend the scheduled reinspection (as specified on this Notice) it is his/her responsibility to secure a different inspection date and time with the Inspector, and provide all tenants with notification as required by California Civil Code Section 1954 (San Francisco Housing Code Section 303(b), if any dwellings, apartment units or guest rooms are to be accessed during the reinspection.

-Please provide access to all common areas of building on re-inspection date in order to perform a complete routine inspection of the property.


ALL ITEMS MUST BE COMPLETED WITHIN 3 DAYS. REINSPECTION DATE: 13 February 2015 10:30:00 AM

IT IS RECOMMENDED THAT THE OWNER/OWNERS REPRESENTATIVE CONFIRM REINSPECTION DATE/TIME.
